I want to know more about the Income Certification Documents (Shoni Iryohi Josei Seido) required to apply for children’s medical expense subsidies?

Answer

Note that this subsidy system has income restrictions for applicants (parents/guardians) with children over the age of one.
In principle, the applicant is whichever parent has the highest income (the breadwinner).
Income Certification Documents certify the income of the applicant, the number of dependents, and the deduction amounts (which are required to decide whether you fulfill the qualifications for subsidies). (Refer to FAQ12855 for details on maximum income limits.)

1. Income Certification Documents include any of the following.

A. Copy of Special Collection Amount for Resident Taxes (Tokubetsu Choshuzeikaku Kettei) (Showing Salary) / Amendment Notification (Henko Tsuchisho) (Original Not Required) (Limited to Those Whose Entire Income is Comprised of Employment Salary) (Documents received from the workplace.)
B Copy of Tax Amount for Resident Tax / Tax Notice (Nozei Tsushisho) (Original Not Required) (The city will send this document to people operating their own business who live in the municipality on January 1 of the certified tax year.)
C. Resident Tax Certificate (Kazei Shomeisho) (Issued by the city you resided on January 1 of the certified tax year.)
D. Resident Tax Exemption Certificate (Hikazei Shomeisho) (Issued by the city you resided on January 1 of the certified tax year.)
Copies of these documents are acceptable. Submitted documents cannot be returned.
We do not accept tax certificate slips (genzenchoshu hyo) or tax returns (kakutei shinkoku) as Income Certification Documents.
The items and names of items certified in the tax certificates (kazei shomeisho) will differ depending on the issuing municipality. A breakdown of income, deductions, and the number of dependents are required.)
If you have children between fourth and sixth grade in elementary school and were exempt from paying city tax, prepare documents showing your exemption status. Note that we will be unable to provide the total amount of subsidies for medical insurance costs if you submit items that do not show your city tax amount or childcare allowance certificate.

2. Tax Year for Income Certification Documents

A. For Children Between 1 year old and Sixth Grade in Elementary School
The target tax year for review differs depending on the duration of the medical certificate you apply for. Prepare Income Certification Documents for the corresponding tax year.
Note that, in principle, the medical certificate is valid starting on the date of relocation, or on September 1.

(1) If applying for medical certificate for September 1, 2017 – August 31, 2018 (*), income calculated for 2017 (2016 fiscal year, January 1 – December 31, 2016) will be reviewed.

(2) If applying for medical certificate for September 1, 2018 - August 31, 2019 (*), income calculated for 2018 (2017 fiscal year, January 1 – December 31, 2017) will be reviewed.

*Medical certificates for children between third and sixth grade in elementary school are valid until March 31, 2019.

B. Junior High School Students
The tax year targeted for review differs depending on the period of hospitalization. Prepare Income Certification Documents for the corresponding tax year.

(1) If applying for subsidies for hospital fees between April 1, 2017 and June 30, 2017, income calculated for 2016 (2015 fiscal year, January 1 – December 31, 2015) will be reviewed.

(2) If applying for subsidies for hospital fees between July 1, 2017 and June 30, 2018, income calculated for 2017 (2016 fiscal year, January 1 – December 31, 2016) will be reviewed.

(3) If applying for subsidies for hospital fees between July 1, 2018 and June 30, 2019, income calculated for 2018 (2017 fiscal year, January 1 – December 31, 2017) will be reviewed.
