How do I enter into national health insurance?

Answer

You can enter into national health insurance through the following method. Note that the required documents vary depending on the situation. For foreign residents, the required items for submission differ. Refer to Procedure for Foreigners to Enter and Withdraw from National Health Insurance and National Pension.

1. Submission Period: In principle, within 14 days of circumstances occurring.

2. Inquiry Window: Residing Ward Office, Civic Affairs Section (Kuminka), or Branch Ward Resident Center

3. Submission Method: Submit directly at the window.

4. Reception Time: Monday – Friday 8:30 a.m. – 5:00 p.m.
Second and fourth Saturdays: 8:30 a.m. – 12:30 p.m. (Ward Offices Only)

5. Closed: Saturdays, Sundays, holidays, and December 29 – January 3 (Ward offices are open on the second and fourth Saturdays).

6. Required Items for Submission
(1) When Moving from Outside of the City
A. Notification of Change of Residence (Tenshutsu Shomeisho) from Prior Residence
B. Seal
C. Documents Verifying Your Identity (Driver’s License, Passport, etc.)
D. A Kanagawa Prefecture Resident National Health Insurance Card (if you have already entered into national health insurance within the same household of the new address)
(2) If Withdrawing from Health Insurance of Workplace (Including End of Free Continuation Period, Loss of Dependent Status, Etc.)
A. Certificate of Forfeiture of Workplace Health Insurance (Shikaku Soshitsu Shomeisho)
B. Seal
C. Documents Verifying Your Identity (Driver’s License, Passport, Etc.)
D. My Number Card of My Number Notification Card (For the Head of Household or for Those Entering into National Health Insurance)
E. A Kanagawa Prefecture Resident National Health Insurance Card (if you have already entered into national health insurance within the same household)
F. Documents Verifying Pension Type, Period of Entry, Such as a National Pension Certificate (In the Case of Medical Care for Retirees)

(3) When Child is Born
A. Health Record Book of Mother and Child
B. Seal
C. Documents Verifying Your Identity (Driver’s License, Passport, etc.)
D. A Kanagawa Prefecture Resident National Health Insurance Card (if you have already entered into national health insurance within the same household)

(4) If You Can No Longer Receive Public Assistance
A. Notification of Suspension or Annulment (Haishi / Teishi Kettei Tsuchisho)
B. Seal
C. Documents Verifying Your Identity (Driver’s License, Passport, etc.)
D. My Number Card or Notification Card (Of All Family Members)
E. Kanagawa Prefecture National Health Insurance Card (If You Have Already Entered Into Health Insurance)

*If a non-family member, or someone other than the applicant is registering, a letter of attorney and documents verifying the identity of the applicant are required at the window.

7. Regarding Issuance of Kanagawa Prefecture National Health Insurance Cards
The window will issue cards for applicants who do not have photo ID.
Note that if you do not have a photo ID, we will send the insurance card via post at a later date in order to confirm your identity.
