COVID-19 Vaccine Booster Shots (3rd vaccine dose)

Eligibility

Those aged 12 and older who have already received their second dose of vaccine, and who wish to receive an additional booster shot

・Pfizer vaccine:People 12 years of age and olde
・Takeda/Moderna vaccine :People 18 years of age and older (Efficacy and safety of booster vaccinations for people under 18 years of age are not yet clear.)

When to get a booster shot / how many doses are required

One dose should be given at least 5 months after the second dose.

Vaccination fee

COVID-19 vaccinations are free of charge

Mailing of the Vaccination Ticket

Vaccination Tickets will be sent out approximately 5 months after the second vaccine dose

Please contact the Vaccine Reservation Call Center in case your voucher is undelivered or you happen to have lost it

What to bring with you when you go to receive your booster shot

Bring your Vaccination Ticket, ID, and Pre-vaccination Screening Questionnaire form in Japanese

1 Receive your Vaccination Ticket

2 Make a reservation

・For those who have a primary care doctor

Consult with your primary care doctor on your booster shot reservation

For those who do not have a primary care doctor, or those whose doctor cannot administer vaccines, etc.

You can receive your booster shot at mass vaccination sites.

To make a reservation, please call the reservation call center. (Phone: 0120-654-478)
You can also reserve through the website below, but the website is only available in Japanese.

3 Read the instructions and fill in the Pre-vaccination Screening Questionnaire form in Japanese

A Pre-vaccination Screening Questionnaire form in Japanese will be enclosed in the mail with your Vaccination Ticket

4 Receive your booster shot on your reservation day

On the day of your vaccination, please bring your Vaccination Ticket, ID, and the Pre-vaccination Screening Questionnaire form in Japanese

 

To receive your first and second dose of COVID-19 Vaccine

If you are unvaccinated and wish to receive your first and second vaccine dose, please consult with your primary care doctor

If your primary care doctor cannot administer vaccines, please call the Reservation Call Center

Reservations for the first dose of vaccine can be made at the Reservation Call Center

Reservations for your second vaccine dose should be made at the medical institution or vaccination site where you received your first dose
