I want to know the procedure when moving to outside the city (including overseas).

Answer

When moving to outside the city (including overseas), you will need to submit a moving-out notice. Once the moving-out notice is submitted, a certificate of moving-out is issued. Then, bring it to your new municipality office within 14 days after moving in. (When moving overseas, the certificate of moving-out is not issued.)

1. Documents to be submitted: A moving-out notice (Resident Moving Out Notice)

2. Submission Period: As soon as possible after the moving-out is decided

3. Where to Submit Notifications: Ward Residents Section of the ward office/Citizens Center at the branch office of the domicile of applicant

4. Notifier: A person themselves or a householder (In case of where a representative submits, the following documents are required.)
(1) In the case that a legal representative (parents / a guardian of a minor / a guardian of an adult, etc.) submits: A family register or other documents that prove his/her qualification as a legal representative
(2) In the case that any person other than a legal representative submits on behalf of him/her: A letter of attorney

5. Submission Procedure: Submit notifications directly at a counter of a ward office or by mail, etc.

6. Operating hours: Monday - Friday, 8:30 AM-5:00 PM
The 2nd and 4th Saturdays, 8:30 AM-0:30PM (Citizens’ Centers of branch offices are closed for the reception on the 2nd and 4th Saturdays.)

7. Closed: Saturdays (excluding the 2nd and 4th), Sundays, national holidays, Dec.29 to Jan. 3. (Citizens Centers of branch offices are closed on the 2nd and 4th Saturdays also.)

8. Required Items:
(1) Any of the following identity verification documents
a) My Number Card / Basic Resident Registration card (with a face photo) / Any License, Permission or Certificate of Qualification which are issued by public agencies such as Passport or Driver's License (with a face photo attached only) / Resident card / Special Permanent Resident Certificate (for a foreign national).
b) Any two items that confirm your name: Health Insurance card / Cash card / Bankbook, etc.
(2) National Health Insurance card / Long-Life Medical Care Insurance Card (members only)
(3) Long-Term Care Insurance card / Aged Recipient Proof card (Only those who hold those cards)
(4) Children's Medical Care card / Medical Care Certificate for Single-Parent Household (Only those who hold those cards)
(5) My Number Card (those who move overseas)
(6) Citizens' card / Seal Registration card (only those who hold those cards) [After the moving-out notice is confirmed, the seal registry in Kawasaki City will be deleted.]
(7) In case of where a representative submits, the following documents are required.
a) In the case that a legal representative (parents / a guardian of a minor, a guardian of an adult, etc.) submits: A family register or other documents that prove his/her qualification as a legal representative
b) In the case that any person other than a legal representative submits on behalf of him/her: A letter of attorney
(8) An unregistered seal (It might be needed for procedures except the Moving out notification.)

9. Note: Besides, you may be required to prepare the following certificates with the old address. (The reception is closed on the 2nd and the 4th Saturdays. We are open on weekdays only for the reception.)
(1) A certificate of recipient qualification (those who are authorized to receive care) [Nursing Care Benefits Office, Elderly and the Disabled Section at ward offices, Nursing Care Benefits Office of the Health and Welfare Station of the District]
(2) A co-payment category certificate (those who hold a medical care certificate) [National Health Insurance and Pension Section at ward offices, Support Desk of National Health Insurance and Pension of the Health and Welfare Station of the District]
