I would like to find out more about the medicala expense subsidy program for children (shoni iryohi josei seido).

Answer

Shoni Iryohi Josei Seido (the medical expense subsidy program for children) is a program that subsidizes the copayment of their medical expenses for children from the age of 0 years old to 3rd grade in junior high school who have health insurance and so on living in Kawasaki City until they graduate from junior high school (excluding the standard coverage of dietary treatment). However, to children who is 1 year old or older, there is restriction on income of their parents. (Please refer to FAQ40189 for more details.)
Additionally, this program has different scope of subsidy by age of children.

1. Those who are eligible: 0 years old to 6th grade of elementary school
(1) Scope of the subsidy:
It's for copayment of hospitalization and outpatient treatment (excluding the standard coverage of dietary treatment). We provide a medical care card for persons eligible.
If your child is out of scope for a medical care card, you can receive the subsidy for hospitalization medical expenses after Jan of 2019. Please see “2. Who are eligible: 1 years old to 3rd grade of junior high school” for the details.
(2) Items required for the claim
a) Kenko Hokenshou (health insurance card) for your child
b) Seals (inkan) (using a stamper with vermilion ink (shuniku)) for an applicant and a partner each
c) Proof of income (required when the child is 1 year old or older. Please refer to FAQ40184 for more details.)
d) Individual Number documents (Individual Number Card or Individual Number notification card) for an applicant and a partner each
e) Documents for identification:
When an applicant (person who has higher income) apply:
The Identification documents for the applicant.
When a representative apply:
The Identification documents for the representative.
f) If a representative apply for a medical care card, a letter of attorney from an applicant or documents that can prove he/she is a representative, such as the applicant health insurance certificate.
*Documents for identification:
・Only one document needed: Individual Number Card, Driving License, Residence Card, Special Permanent Resident Certificate, Psysical Disability Certificate, Mental Disabitiry Certificate, Intellectual disability certificate, Passport, so on.
・Two documents needed: Health Insurance Card, Pension Book, Bank Book, and so on.

[How to examine the income]
・If the child is 1 year or older and your income was confirmed outside Kawasaki (you lived outside Kawasaki on Jan 1st of the year), there are 2 ways to examine your income: 1) Inquiring your income data based on Individual Number on the application form. 2) Submitting the document which can prove your income. (If your income was confirmed in Kawasaki, we use your tax payment information to examine your income.)
・If you moved from outside Kawasaki or an applicant (a guardian) is living by himself/herself away from the family in other city, submitting the document which can prove your income is not needed by using Individual Number for the inquiry.
・Because the way to use Individual Number for the inquiry takes relatively long time until getting the answers form the cities (local governments imposing a tax), we cannot issue a medical care card on the same day in principle. If you would like to receive it on the same day, please submit the document which can prove your income.
・If you have not submitted the tax return form, your reference for your tax payment information is wrong, or you have no income, we cannot get the right answer. In that case, you should submit the document which can prove your income.

(3) How to use the program
After a qualification screening, Iryoushou (a medical care card) will be provided after certified.
When visiting a doctor, please show Iryoushou (a medical care card) with Kenko Hokenshou (health insurance card) at the reception counter of such medical institution.
Generally, you do not need to pay doctor's bill in principle, except the upper limit of the copayment for grade 4 – 6 elementary student.
*Even If the child is a grade 4 – 6 elementary student, the applicant (guardian) is subject to Municipal Residence Tax (an income-based portion) exemption, the copayment of Insurance medical expenses is also subsidized (you don’t have to pay at the counter of the medical institution).

(4) Service Counter to claim:
Support Desk of NHI/ Medical Expense Subsidy of National Health Insurance and Pension Section of each ward office and NHI and Pension Support Desk at Citizens Center of each branch office

(5) Operating Hours:
Monday - Friday. 8:30 am - 5:00 pm
The 2nd and 4th Saturdays (only at the ward offices), 8:30 am - 0:30 pm

2. Those who are not eligible: 1 year old to 3rd grade of junior high school
(1) Scope of the subsidy:
It's for copayment of hospitalization (excluding the standard coverage of dietary treatment). (What is not covered by health insurance such as bed fees and documents fee is out of the scope of the subsidy.)

(2) Items required
a) Kenko Hokenshou (health insurance card) for your child
b) Seals (inkan) (using a stamper with vermilion ink (shuniku)) for an applicant and a partner each
e) Bankbook of the applicant or any document that shows the applicant's account number.
d) Receipts issued by medical institutions
e) A grant notification for high-cost medical care expenses or the subsidy for family therapy and so on (only applicable)
f) Proof of income (required when the child is 1 year old or older. Please refer to FAQ40184 for more details.)
g) Individual number documents (Individual Number Card or Individual number notification card) for an applicant and a partner each
h) Documents for identification:

*There is no upper limit of income, but this program is subject to the subsidy of Kanagawa prefecture. We would appreciate if you submit the documents or Individual Number we can use to check and confirm your income.

(3) How to use
Iryoushou (medical care card) is not issued.
After paying a medical bill to a medical institution, please claim as soon as possible on or after the 1st of the following month of the month that you see a doctor.
We will transfer the amount to the account you designated around 20th of the next month when you claim.
(For the bills after April, 2017)
Please claim as soon as possible in or after the following month of the month that you see a doctor. It becomes invalid after 5 years from the day when you visit a doctor, and it won't be able to be subsidized.
(For the bills before March, 2017)
We cannot subsidize them because 1 year past.
* There is a upper limit of income for the subsidy of the hospitalization expenses before December, 2018.
